Our Insurance Disclaimer
As a courtesy, we are happy to bill your dental plan for services. However, when we call on your insurance and verify benefits, it is not a guarantee of payment by the insurance company and may vary according to your individual plan when the actual claim is submitted.
Any treatment plan that our office proposes to you is an estimate of what your insurance coverage will be; it is not a guarantee. If you need exact payment of benefits, then a pre-treatment is required. If you would like this done, you must notify our office staff before any work is initiated as this could take approximately 6-8 weeks to be processed.
Please remember that the contract itemizing your dental benefits is between you, your employer, and your insurance company. Regardless of coverage, your estimated co-payment is due in full the day of treatment.

We want every patient to have a full hour or more with our Greater Fort Lauderdale dentists which is why our office upholds a strict broken appointment policy. Because of this, we ask for at least a 48 hour notice (or 24 hour depending on your visit) prior to your scheduled appointment if you must cancel or reschedule.
Appointments are in high demand and your early cancellation will give another person the possibility to have access to timely care. If you need to forgo your time lot, you can:

If an appointment is not canceled at least 48 hours in advance you may be charged a cancellation fee!
First canceled/missed appointment without advance notice:
We do understand that unforeseen situations happen. As a courtesy, we may waive the first cancellation/missed appointment. However, that appointment must be rescheduled within 30 days.
Second cancellation/missed appointment without 48 hours advance notice:
You will be required to prepay in full for future treatment and any outstanding balance to secure another appointment, requiring a credit card on file.
Third cancellation/missed appointment without 48 hours advance notice:
Third missed appointment may result in *full visit fee, depending on dentist/visit, and dismissal from the practice.
Cancellation/missed appointment fees:
- Less than 48 hours: $79 (hygiene appointments)
- Less than 24 hours: $100/$150/$200 (dentist appointments)
